Student Application Forms

Posted September 26, 2026

Student Application Forms is the Campus list of every household asked to sign a student's Application Form: one row per student and household, with where the signing stands, the invoices it posted, and the signed contract. It is found under Finance > Application Forms > Student Application Forms and requires the Contract Full or Contract Read role.

Choosing what to list
- The form list, Select an application form, offers the student invitation types of every Application Form, newest first. The chosen form is kept for the session and shared with Application Forms - Options Selected.
- The status list shows the rows of one status. The default is All Statuses.
- The search box finds a student's last, first, or preferred name, or a household name. Click Search.
- Export downloads every row that the filters show, on every page, as a spreadsheet.

Columns
- Last Name opens the student's Finance profile for users with the FinanceAR Full or FinanceAR Read role. First Name, Id, Grade, and Grade NY describe the student.
- Household is the household that signs the row's contract, and Share is its split percentage, frozen when the invitation was created.
- Status is where the household's signing stands, as described below.
- Started, Parent 1 Signed, and Parent 2 Signed are when the household opened the form and when each parent signed.
- Countersigned is the date the office countersigned the household's contract. It is shown on forms with an office countersign.
- Invoiced, Paid, and Due total the household's invoices that the signing posted, without voided invoices.
- Contract is a PDF link to the household's newest stored contract: the signed copy, or the countersigned copy once the office has countersigned.

The column gear in the grid header shows and hides columns. Pref. Name, Household Id, Countersigned By, Declined, and Received at Signing are hidden by default. Received at Signing is the amount paid when the household signed, for example a deposit.

Statuses
- Not Started: the household has not opened the form.
- In Progress: the household has opened the form, and nobody has signed.
- Partially Signed: one parent has signed, and the household needs a second signature.
- Signed: the household has every signature it needs.
- Countersigned: the office has countersigned the household's contract.
- Declined: the household declined the invitation from the portal.

The status belongs to the household, so the households of a split family can show different statuses. The invitation is Complete once every household has signed. Below the list, the totals give the number of students, households, and Signed households, and the number of Countersigned households on forms with an office countersign.

Countersigning
On a form with an office countersign, users with the Contract Full role also see a check box on each row, a check box in the header that selects every household the filters show, the Selected count, and the Select Multi Action... list with Countersign. Review Application Form Countersigning.

Application Forms - Options Selected
Finance > Application Forms > Application Forms - Options Selected lists, for the chosen form, every option the families signed: one row per selection, with the student, the household, the Selection, the Product, the household's Share, the Amount, whether it is financial aid (Aid), and when it was signed (Signed). It has the same form list, search, and Export, and requires the same roles.

Note: Only students are listed. An accepted applicant's invitation is listed once the applicant is enrolled and the student's invitation is created. A household with a 0 percent share has no contract and no row.
