Application Form Pages

Posted September 26, 2026

Application Form Pages are the CMS pages a family walks through when completing an Application Form, arranged as a site map in the order they are shown. The page is found under Admin > Applications > Application Forms > Form Pages and requires the Website Full role. It uses the same site map editor as CMS - Site Map, limited to one level of pages under the form.

Title is the name of the form's site map. Auto Sort orders the pages alphabetically instead of by the order in the tree. Used By Controls lists the menu widgets that read this site map, normally the CMS Widget - Menu placed on the form's master page. Save stores the title and the sort setting.

The tree lists every page of the form. Each node offers the following actions.

- Add child page opens the Node Information window to add a page under the form.
- Open page in new tab opens the page in the CMS page editor, where widgets are added and configured. Review Adding CMS Widget.
- Reorder top level pages opens a window where the pages are dragged and dropped into their new order.

The Node Information window has the following settings when a new page is added.

Page Name creates a new, empty page with the given name. Copy Page creates the new page as a copy of an existing page of the form, including its widgets. Menu Title is the text shown on the form's menu; it defaults to the page name. Action to PDF associates a CMS form action type with the new page, as on other CMS site maps; it is not needed for an Application Form page.

When an existing node is edited, the window shows the following settings instead.

Url is the page the node points to. Title is the menu text. Open In New Window opens the page in a new browser tab. Dynamic Data Source lets a widget on the page add its own child nodes to the menu; review Adding/Removing Page to Site Map through CMS Widget.

Save and Save and New store the node. Clear Completed Tags removes the completion marks that the menu's Track Completion setting has recorded for the page, so every family sees the page as not yet completed. Delete removes the node from the site map; the page itself remains in the CMS.

Note: The last page of the site map is treated as the signing page. The pages before it are the pages the family fills in, and they are the pages that the signing gate checks for completion. Place the CMS Widget - Sign and Submit widget on the last page only. Review Application Form Signing.

Note: The form's pages are CMS Pages and share the page settings described in CMS Page Details, CMS Page Roles, and CMS Page Filter. Restricting a form page by role or filter hides it from the family; use the invitation type filter to control who receives the form instead.
